Q: how do you lubricate

A: I would start by disassembling the hinge, driving the hinge pin out (loosen the NRP feature first if you hinge is so equipped) and then I would use white lithium grease to lubricate not only the joints between the hinge Knuckles but I would dab the grease on an inside of the bearing raceways as well. You might benefit from using brake cleaner prior to beginning the process. If the condition of the hinge is beyond maintenance and cleaning you may just simply consider purchasing new hinges.
