Q: The closed door sits atop the oak top....or in front of the oak?
A: When the door is in the closed position, the door usually sets over the adjustable oak top - but the exact position will also need to depend on, and take into account, the other conditions of the opening such as the sill condition, flooring condition and other hardware you plan to install on the bottom of the door.

Q: How about a video on how to install one? Sometimes this type of threshold is at the bottom of a new door, but we all know they wear out faster than the door, and you need a new one. No visible screws for the concrete are seen, so just how is it installed on an existing application?
A: There is no actual suggested method to install the Pemko 95518AW Mill Finish Aluminum Adjustable Oak Top Sill Residential Threshold into an existing opening as these thresholds are used a structural members of the wood frame when the doo unit is built and held in with staples or other fasteners and installed as a unit with the door. It is a "pre-hung". this makes extracting the threshold only a guaranteed chore to remove and nothing less then troublesome.
If you are able to successfully remove the exiting threshold I would only suggest drilling through aluminum portion, and inserting screws below into and through the wood substructure and then inserting silicone and plastic caps to cover the newly created holes.
I have had one person tell me they removed adjustable wood top, screwed through this area securing the threshold and then reinstalling the adjustable wood top. I asked for proof of concept but to date am yet to receive.

The upper plastic portion of the threshold has warped. I’m trying to avoid removing the entire threshold by finding out the manufacturer so I can purchase the threshold and part-out the warped component. Can you tell me who the manufacturer is?
A: That manufacturer is unfamiliar to me and I can tell you that it is a very common approach that you are taking by trying to buy entire threshold and cannibalize it for the required parts.
