A: When replacing the door only, it is best to identify the manufacturer of the door and we do this by measuring the existing hinge and lock locations. The chart here:

A: No, a 161 prep is a single cylindrical lockset prep only specifically a 2-1/8" hole at 2-3/4" backset with a 2-1/4" x 1-1/8" edge prep. Nothing more, nothing less.
